Waiver & Release

I have received and will adhere to Dance Quest International, Inc. Policies and Guidelines and will take the responsibility to carefully read and follow the rules and policies therein. I understand that Dance Quest International, Inc. does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc. I further understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my child's participation with Dance Quest International, Inc., and I voluntarily assume the risks associated with such participation.

I understand that even after responsible measures have been taken, course activities may involve hazards for which Dance Quest International is not responsible. If my child or I become ill or injured during class or while on Dance Quest International premises, I authorize staff to seek emergency medical care.

In consideration of being allowed to participate on behalf of Dance Quest International's Dance program and related events and activities, I acknowledge, appreciate, and agree that participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist. I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases.

I understand that Dance Quest International utilizes photos and media in all forms for advertising, trade and any other lawful purpose. If I deny permission to use my child's name and/or photos, I will make a notation on my child's profile. I understand that the Annual Dance Quest International Showcase is professionally video recorded and photographed. By opting out of having my child photographed/video recorded, I will have the option to make the exception to be included in the Annual performance showcase. Should I also opt out of having my child photographed/video recorded during the Annual Dance Quest International Showcase, I understand that I am also opting out of performing in this showcase.

Electronic Signature Agreement. By selecting the "I Accept" checkbox, I am signing this agreement electronically. I agree this electronic signature is the legal equivalent of my manual signature on this agreement. I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by its terms.
